this sounds like a bad blower motor resistor that is bad and only working when the switch is put in the 4th position. the resistor has resistors wires in it that respond to the path of power through fan switch on the dash, depending on which fan speed you need.

Might be the Resistor pack located close to the blower. It has a flat connector with 4 or 5 wires, and two small screws holding in the duct. It may be a crispy critter. It happens... The blower moves air across it to keep it cool. The blower motor may be bad as well, or possibly the fan speed switch. May very well be the fuse for the blower as well. common problem, replace the blower motor (fan) resistor, this is what controls the fan speed,its located on heaterbox next to blower motor.heres a picture of it,its on the rightside of dash toward the bottom. hope this helps.
