I had the same problem on a 03 Avalon. It is the connector that comes with the "generic" sensor you bought. It tests fine, but the computer somehow sees the resistance in the connector. I even cut it out and soldered the connections, still no luck. Took car to a local shop and they said they had the same issue with generic sensors and quit using them. Bought the Bosch sensor with the factory plug (best price is on amazon.com) and problem solved.

this a v-6 if you have 4 senser they are inf front and behind each converter car needs to be lifted up and requires special sockets

Are you sure the O2 sensor is what is causing the light to come on? Code 21 is Oxygen sensor. Sometimes another fault can cause the computer to think it is the oxygen sensor. Such as when you have a misfire, causing the sensor to read rich, or when the MAP sensor is faulty, causing a rich or lean condition.
Also, did you replace the sensor with a new one? Or second hand? Replacing with a second hand sensor is bad practice as they typically have a service life of 100k miles.
If you replaced the sensor with an aftermarket type, it could be the wrong one, thye all have different resistance values for the heating circuit & the computer uses this resistance value to determine if the sensor heating circuit is faulty.
