chances are it's the gasket around the tranny pan. you'll have to remove the pan, gently scrape off the old gasket (without scratching the metal surface), replace with new gasket, replace the pan. put all the screws half-way in, then tighten screws across from each other.
SOURCE: transmission slipping
IF THE TRANSMISSION SLIPS THE FORWARD CLUTCHES ARE LIKELY WORN OUT OR THE TRANSMISSION HAS LOW HYDRAULIC PRESSURE, BOTH REQUIRE THE TRANSMISSION BE TORN DOWN AND REPAIRED OR OVERHAULED.

I have the same problem with my Grand Caravan 2000 model. I fixed it once where it cost me approx $1500. We travel on a lot of gravel and it is Winter here a lot. Because the lines are right behind the wheel on the passenger side (what a silly place for such and important piece). It broke again anyway and now another $1500. Not!
I did find after much searching some parts on StreetRodGuys.com . It is a couple of pieces that screw onto the lines going to the back....thereby blocking the rear air off. We really didn't use it that much anyway...the rear that is. They say that when you recharge it to use about a pound less freon when it is recharged. Part is around $50. I plan on using R12a which is colder freon than R134a or R12 so should work fine. Parts just came this week.
Makes it much more bearable in the heat.
SOURCE: where do I put the transmission fluid in a 2000
You add the fluid in the same place where you check the fluid for the transmission. normally a yellow stick labeled transmission and its under the hood on the driver side of course. 4 thumbs up is appreciated if this helps.
