you probably have a vacuum leak and by the sound of it, it idles rough with the brake pedal depressed its your brake booster

That sounds like a transmission going bad to me.If it is shifting hard/winding out,maybe herky jerky too.With the rough running,truck shaking,and rough idle,it very much sounds it.Check the transmission fluid.Let it run for 5 minutes,leave it running and do at least 4 or 5 dips with the stick.If you see anything other than fluid such as metal chips or flakes,that is a definite indicator.If you do decide to replace the transmission,it is best to buy a new one,if one is available for your year.I have seen many people have disappointing results with used or rebuilt ones.You should call a couple shops and ask for their opinion,get estimates from different shops.NOT AAMCO.Someone is always willing to give a free opinion over the phone because it helps attract business.Sorry you already replaced all that stuff,but if you buy a new tranny,that truck will go another 20 years I bet.Good luck!
