1997 Nissan Hardbody has rough idle/surges/stalls

Thank You in advance for any help you can provide.
Vehicle: 1997 Nissan Hardbody 2.4L(KA24E)4 cyl. 5 speed, 211,000 miles. Motor starts fine but the idle is rough and the engine surges about every 15 seconds or so. Not always, but that's what's happening. Checked all vacuum lines, TPS, IACV-AAC(replaced), all solenoid switches, EGR, BPT valve, changed the PCV and connecting hose , checked all fuel lines for blockage(clear), injectors all check out ok, harnesses all have continuity and battery voltage present, checked the fuel pressure at idle and is: 36-38 psi(should be 34 psi), then the engine will surge(rev up) and the fuel pressure will drop to 34-35 psi(where it should be). When I pull the vacuum line off the pressure raises to 43 psi and stays there until I put the line back on then hovers at 36-38 psi then surges and the motor shakes due to the rough idle. Any thoughts?! Thanks Again!!! I have a 2001 NISSAN ALTIMA with 2.4 litre engine. It is surging badly at idle. It is showing code for random/multiple misfire.I have replaced plugs, wires distributor cap, rotor button, air filter, all to no avail. I have checked all vacumn lines that are visible and see no problem with none of them. Engine smooths out and runs well at all speeds above idle. Any help is grestly appreciated. I am out of ideas.

The 2001 altima is what i have and I had the same problem as you. turned out to be a bad head gasket factory used cheap ones for a few years before they found the problem. one way to test this is to get a can of carborator cleaner and spray it around the area where the head meets the block under the intake problem should be around the 4th cylinder or close to driver side. if the head gasket is bad the rough idle will smooth out while you are spraying the head.

ok i have the same truck my timing was off 3 teeth but mine wouldn't kick out of high idle is y mine was surging but know with timing right no vacuum leaks i cant get it below 1400 but when a vacuum turns on it almost kills my engine any thoughts i know i got a vacuum line mixed up idk were though

2.4 L KA24E I4 (1990-1997)
in the USA the EPA banned all those screws.
by law. the idle speed is fully computer controlled.
all USA cars, idle speed is regulator fast by the PCM
using device called IAC/ISC/ has 2 wire IAC.
IAC is bolted to TB lower side base, 2 wires.

if Idle hot is not at 800RPM and not regulating there are 3 common causes.
  1. ISC is stuck (needs cleaning)
  2. vacuum leaks, makes the ISC useless. (over-ranged) cracked vacuum lines and more.
  3. thermostat bad,engine never gets to 180f up. temp.

the screw here is factor OEM calibration
called the never touch screw.

yellow circled. this screw sets idle duty cycle to 50%
as covered in the real service manual
seem carb guys monkey this screw and IAC and PCM go nuts.
if was messed with by others I set mine to
0.001" gap TV to bore inside the TB bore'/

When driving in 5th gear should the RPMs run at 3000 RMPS. Mine is a 4 cly 5 speed

You don't provide much information to go on (type of vehicle, type of engine, 2wd or 4wd, etc.) RPM is always going to be based on the speed you're traveling with a standard transmission. My 4 cyl 5 speed Nissan pickup in 5th gear runs at 3000 RPM at about 75mph.

Have a 96 camry 4 cyl auto. After highway driving, the idle becomes rough and wants to stall. The problem eventually goes away after driving a city speeds. Around town, it runs all day just fine. Help ,...

Here is the most common cause of surges and stalls and low idle, it is the idle speed control air-bypass valve and throttle valve, they get full of gunk over the miles and cause idle issues (stalls) like yours, Get a can of intake cleaner from any local parts store, not carb spray, intake cleaner, it is made by a company called CRC, remove the air intake hose to the engine, hold the idle high so the engine won't stall, then spray the can of cleaner into the intake while keeping the engine running, use at least 1/2 the can, shut down the engine and disconnect the battery for 5 minutes, then restart and complete a number of mixed driving cycles, town, freeway, stop and go etc., after a few days the problem will go away as the system will relearn to the clean intake.
