the idle surges if this is the case take out the air intake tube that leads to the throttle body, get a can of carburetor fluid and spray around the throttle body. You will need someone to start the car and hold the gas peddle to 2500 rpm while you do this the car will shut of when you do this, restart the car and repeat the process until half of the can is used up. Now check your antifreeze level fill if needed.
There is a warm up valve on the throttle body(should have 2 small coolant hoses). remove the valve and have someone start the car while you block the port to the valve on the intake. If that clears up the problem you can cut a piece of cardboard to mount in between the valve and intake.should straighten it out.
Check your 91 Accord's ignition coil. The coil goes bad at apprx. 150k miles and cause the car to run rough, hard to start(long cranking time) in the morning, stall after the vehicle has warmed up, cut out while driving and backfire horribly when accelerating from a standstill.
Something is riching or leaning this vehicle out at the wrong time. I'm gonna go with ERG valve being stuck in the open position. The only time this thing should work is at crusing speeds. Once the car has cooled down the fast idel cam takes over and prevent it from stalling. Once temp is within range idle cam stops and EGR is dumping into the mixture.
Use shop manual to do proper testing for the valve( probably a hand held vacuum pump is all you will need and gum out to free it up but you should properly replace it if it turns out to be the culprit.
