Idle rough in R, D, fine in neutral

Hey gang, I have a 99 Mazda 626, 4 cyl, and have an annoying issue I am dealing with. When the car is in drive, L2, L3, or reverse, but not moving, the idle is rough. Not to the point of choking out, but rough nonetheless. When I put the car in neutral and park, it's smooth as a baby's bottom, and so quiet you don't even know the car is running. Someone told me it might be the mass air flow sensor, but it's when the car is not in motion - does that sound right?
(I've already done a tune up, changed plugs and wires as well)

    Car will stall sometimes when you are slowing down to stop (automatic down shifts and then rpms drop car shuts down). I have replace IACV, mechanic saying transmission needs replacing @ 3,100.

Hmmm have you had the timing belt changed? is it due for one? that can cause a some rough idling.

check and or clean the (IAC) idle air control, this is what controls idling on most of todays cars and light trucks. if iac is removed for cleaning. clean out where it mounts to engine or fuel injection unit. good-day !

check connection to cam sensor. sometimes corrosion will cause a poor electrical connection. clean as needed. otherwise you may have to replace the sensor.

I suggest a tune up, also replace the fuel filter and air filter, the timing is possibly off slightly causing the engine to run rough. Have the timing checked to make sure its not just slightly out of time first

Looking for oil filter position on 99 mazda 626

On my 98 626 4 cyl, 2.0 it is located on the back right side of the engine underneath the intake manifold. You can reach it from the top if you have slim arm and small hand. Best reached from underneath.

99 mazda 626 loses power after starting. It sometimes will run rough but not all the time.

Car stalls while driving at high speeds

i would start by checking the spark plugs and leads. if that ok i would think there is a problem with the dispack that the spark plug leads fit to, if its cracked or any rust i would replace.
