In extremely cold weather, the fan makes a loud "almost grinding" noise for several minutes. The noise sometimes ends when you make a sharp turn and/or the motors been blowing for a few minutes (i.e the car heats up). My first thought is that something inside the blowed is stuck or frozen up or that its a vent issue in the heater. The heater itself works fine once you get beyond the noise issue.

6 screws to remove glove box, One plastic nut on kick panel and 3 screws hold the blower to the air box.
I had this same problem. It got cold and a mouse built a nest in the fan. I drive the car almost every day and its garage kept. After taking out the mouse and its nest the noise was gone.

I have the same problem (02 Maxima GLE with 125,000 miles). To me it sounds like something in there just needs to be greased, but I have no idea how to get in there and do that. I hope someone has a solution.
I have a very similar problem on my 2003 Maxima. Originally thought it was the fan blade, but now I believe it is associated with the dampers. Worse in cold weather with cold engine and seems to be initiated by acceleration or bumps in the road. When it stops, everything seems to work normally.
