Between ! RPM and 2 RPM, the car has a rough idle and it requires more gas to get it to drive. Very slow start than it picks up. Some times it drives okay{ especially when I take off after I crank it up ), than it struggles.
Sounds like an air intake problem, when u first crank a car it running in a choked state so until the engine warms up there is actually little air being up into the system.. Now once it warms up, this is when it begins to get more air into the system and I'm thinking when your problems arise.. Cheapest/Easiset things first: 1. Change your air filter 2. Clean all sensors between the filter and the throttle body, then the throttle body itself.. 3. Run a (pour-in-tank)system cleaner thru your gas tank and take a long trip at sustained a sustained feed. Do this when you plan on doing a road trip and can drive out at least 3/4 of the tank at one time.. Hope this helps you..
Could need tuneup and maybe change fuel filter ,,this would be a good start....

Usually the fuel pressure regulator vac. hose pops off? If not listen for a slight whistling noise (vac leak).

4.9 litre ?? you having a laugh its a 10ton truck engine not a pick up -global warming machine,remove the pump/sender unit and check the gauze filter also look inside tank for a something floating around ,i think the idles is not a related problem to the vehicle losing power but their is a possibility you could have a problem with the little vac operated thing on the fuel injector line that allows the fuel through only when the engine is running

Check your fuel vapor recovery system, your van has unusual set up, and the effect is RAW fuel gets purged, rather than vapor.
The dead giveaway test is does the issue happen on light acceleration? Does it stop if you let off the throttle? Can you accelerate harder and "get past" the issue?
