Transfer case encoder or shift motor.just cause it moves doesn't mean it is working. It works off of specific voltage to pick position. Grab one out of a junkyard.

That code is for an encoder motor(shift motor) on the transfer case.Not difficult to change but expensive to buy(200 dollars).Check all the wires and make sure none are nicked and that everything is plugged up good before replacing.You will need to remove front drive shaft to get the encoder motor off the transfer case.The hard shifts are due to a limp condition set by the code being stored.

There could be many causes but based on your vehicle, I would start with the electric shift motor. It is not uncommon for the shift motor to fail or freeze up. Sometimes it just boils down to lack of use. The electric shift motor, mounted externally at the rear of the transfer case, moves the transfer case from 2WD, 4WD HIGH, and 4WD LOW. It should be located on the driver's side of the transfer case. While parked with your transmission in neutral, have a friend watch the shift motor while you switch from 2WD to 4WD and back. The movement of the cam on the shift motor should be visible as well as audible. If no movement, in some cases it can be manually assisted by hand. Just a place to start.

This vehicle has an "electric" transfer case shift motor.... I've seen them fail,... I've seen them with bad connections..you will need to verify function of this motor, and/or control signal going to it being a 2000, I assume a the truck has some mileage, my bet would be bad transfer case motor

Your transfer case could be locked up. Try pulling the shifter motor (servo) off of the transfer case and manually shifting the vehicle into 4x4. Make sure to block the wheels as it will roll if you shift into neutral. If you cant shift it, you need to have a professional look at the insides of your transfer case.
