I have a 1993 f 250 351 5 spd the engage on the clutch was always really high in the pedal. now the truck shifts smoothly into all 5 gears and reverse but when driven the engine revs but barely touches the wheels have to drive at 3-4000 rpm. is this typically a clutch or transmission issue?
Many consider it a clutch problem but expensive experience shows that you should be replacing the clutch master cylinder first and then the slave cylinder before committing money for the clutch kit
It is normally a blocked compensating port in the master cylinder that prevents the return of fluid to the reservoir when letting the clutch out and so the clutch is only partially engaged
If you persist in slipping the clutch ( 3-4000rpm) to go any where then it will quickly become a clutch replacement problem along with needing the heat cracks of the flywheel to be machined out
Sounds like worn clutch disc.

You might have to try to adjust the freedom of clutch pedal,
The noise during shifting can be adressed to exessive wear clutch parts or again it might be a sign that indicates the not complete enabling the calutch
To find out what is causing the noise, set the parking brake, place the vehicle in neutral and start the engine.
If you hear growling or grinding noises when the clutch is engaged, the cause is the transmission input shaft bearing.
A squealing sound that occurs when the clutch pedal is depressed and held is usually caused by a bad pilot bearing or bushing.
A chirping noise that intensifies when the pedal is slowly depressed would indicate a bad release bearing.
If you hear chirping while idling in neutral and the noise goes away when the pedal is slowly depressed, the fork/pivot ball contact point is making the noise
Hope that helps.

SOURCE: E40d trans will not shift into 3rd or 4th gear, only rev. 1+2
check or replace your thottle position sensor tps that trans needs to know where the throttle is to shift if it is bad it will do as you say wont shift to third
another telltale is it will act like it goes into lockup when shifting into reverse and stall the engine
(electricity) An electric switch that is connected to the ignition switch of an internal combustion engine and prevents starting the engine unless the transmission shift lever is in the neutral or park position, or the clutch pedal is depressed.
Might try tilting the steering wheel up or down and starting in different positions, maybe loose wire in tilt.
