Yes clutch fan could be bad,Radiator could need flushing,could have blown head gasket.To check clutch fan use a broom handle to see if fan will stop rotating when running at operating temperature.Not something you should do yourself as it takes practise to do without damaging fan if its still in good working order.If fan is not working very well this test shows it to be faulty very quickly as therse no resistance to the broom handle trying to stop it.To check if head gasket is an issue a TK Head test will show it up very quickly.Extremely accurate test.
