If your vehicle has one, and I think it does, check your EGR system. If the egr valve is full of carbon, it can be partially open, creating a vacuum leak. If the egr valve solenoid is bad, it could be opening the valve at the wrong time, also creating a vacuum leak. Unplug the egr vacuum hose, plug the open end of the hose and start it and see what it does. A picture of the egr is below.

Check your fuel pump presure. Pump is in the tank. Also try and loosen the fuel cap, the tank vent may be stopped up.

Doesn't sound like you've considered spark. Sounds like you're losing spark somehow. You seem to know your way around a truck, so I don't have to explain every little thing. Just check all the ignition stuff, go through the distributor and coil and plugs and wires. If it's gettin fuel, its either a spark issue or air/vaccum issue. Hope this helps.
Engine stall or RPM fluctuates
at idle.
This problem
is commonly caused by a dirty automatic idle speed control valve and
throttle valve. Buy a can of throttle valve cleaner (do not use
carburetor spray cleaner!) from NAPA or Carquest (made by CRC
chemicals) and spray it into the air intake while the engine is
running, use up about 1/2 the can, engine will try to stall hold the
speed up, shut it down and let it soak for 30 minutes, restart and
blow out the remaining fluid, shut it down and disconnect the
negative battery cable for 5 Min's to reset the base idle control.

replaced ignition module, no change. replaced coolant sensor, no change. it is definitely a cold start problem. it starts right up and after a few seconds of normal it starts to surge( revs up and down between 700 and 1200 rpm). in gear it has no acceleration power except from idle rpm. as it warms up it has a more steady rpm and begins to respond to the gas pedal until it is finally running perfect. checked for short on plugs and wires, seems good.
also smelling fuel right after startup. is it possible to not have full spark at startup but perfect spark after warmed up.
checked spark plug wires, good strong spark to plugs. installed new mapp sensor , no change. still runs rpm up and down until vehicle is warmed up. then runs fine.
