There was a recall for the tensioner pivot bolt for the 2006 Civic EX. Service bulletin 09-007, Part# HON 04301RNA405.
Part cost $11. Service $42.50.
Cost covered by Honda even with my car being out of warranty.

There may be a recall for the non-EX model also.
I have the same car here in Ireland but we call it the Civic 1.8 SES (assembled in Japan) and the belt tensioner bolt broke and had to be drilled out of the engine block. Three things to check.... that you are putting on the exact specified part for that motor. second that it is being installed correctly... third that you are using the correct belt for that motor.... oh and a forth.... make sure that you are correctly routing the belt on all the pullies.
There should be a sticker with the correct routing of the serpentine belt either on the lower part of the engine compartment.... or look up at the hood when you have it open.

Look on page 215 of your owners manual and it lists out most of the information you are requesting. the B signifies an oil change and several other inspections of the vehicle. The 2 indicates replacement of air filters and inspect drive belts. the 1 indicates a tire rotation. The 5 indicates coolant replacement. The 8 has me confused. Make sure that it is not another number like a 3 or a 4.
Before removing the timing belt bring the engine to the timing marks and if your engine is working ok,then use the marks by using some reflecting paint . Loose the timing tensioner and remove the belt. fitting the new belt and tensioner be careful on the installation the marks to be on the marks and fit it. release the tensioner and tight it. Turn the engine by hand twice and re check the timing marks.fit the covers crank pulley and mounting and start the engine.
its more than likely the alternator drive belt needs re tensioning if its got aircon that belt may cause the same problem just get the drive belts checked and re tensioned
You will need a long handled box wrench on the belt tensioner from above the engine. Turn the wrench slowly in the direction shown in the pictures below. With the tension released remove the belt.

The water pump is located underneath the timing belt cover, on the drivers side of the engine. To swap the water pump, you need to remove the timing belt cover, timing belt, and the timing belt tensioner. I usually tell people that this can be done in a day, but sometimes not ! Also, all of the other drive belts , such as Air conditioning and the alternator belt all need removed too.
Place a long-handled, boxed-end wrench or a belt tension release
tool (A) on the drive belt auto-tensioner from above the engine. Slowly
turn the wrench in the direction shown to release the tension, then
remove the drive belt.
WARNING
This is a hydraulic type auto-tensioner; you must turn the wrench slowly.
Install the new belt in the reverse order of removal.

First, note how the drive belt is routed, then use a wrench on the tensioner pulley center bolt and turn the tensioner clockwise (four cylinder engine) or counterclockwise (six cylinder engine) to release the drive belt tension. Remove the drive belt from the tensioner and relax the tensioner. Fit the new belt on the other pulleys, then turn the tensioner again and fit the new belt over the tensioner.

The alternator drive belt is loose or worn out. If the alternator is driven off a serpentine belt then replace the belt. Inspect the serpentine belt tensioner and replace if necessary. On an 18 year old car it is more than likely defective. If it is driven off a standard drive belt then adjust the alternator to tighten the belt. Inspect the belt for fraying and or cracks and replace if you see any.
