Possibly a bad ignition coil. I just went through this issue. Most coils just fail. I have now seen one that was putting out very weak spark. The spark was a white color when I had a spark plug hooked up to it. It should be a hot orange or blue spark. The spark was too weak to fire under load conditions. Good luck.
1990 DODGE W150 5.2L 318 V8 TBI CKP SENSOR WTF
I looked at wiring diagram for your year and engine size, It didn't show a crank sensor. I believe you do have a cam sensor in your distributor. If that is the problem? My info on your model year isn't great.

Get the fuel injectors and ignition cylinders and fuel pressure inspected.Injectors going bad due to heat ans worn out.Get the compression test done for each cylinder.---- This will confirm which cylinder is faulty causing the problem. If the fuel pressure is low or fuel line is clogged dirty at fuel fuel filter , then it can cause the problem.------------
Also get the crankshaft sensor and cam shaft sensor checked. The sensor located in distributor.-

Getting this possibilities checked will help.Thanks.Helpmech.

The engine computer is also the voltage regulator.
I copied the following from another site. It might be something you should look at.
"The voltage regulator is built-in to the ECM...about a $275 part! However, before you jump to conclusions about the voltage regulator being defective, I'd highly recommend that you go visit the autozone website and follow the "Check charging system" word for word. I discovered that the alternator in my 1990 Dodge van wasn't grounding? How could that be? It's bolted to a metal bracket, right? Could be corrosion, could be rust? I even installed a second ground strap from engine to body. Nothing worked until I ran a ground wire from the battery to the alternator case. Charged just great then! Of course I discovered this after first replacing the alternator and the ECM! Don't you make the same (expensive) mistake."

90 dodge w150 won't start. It is acting like out
Sounds like firing order or you have moved 1 cyl up or down
on cap
Rule of thumb take out no 1 plug and take off cap put your thumb
over plug hole so you can feel compression stroke if you have timing
marks line them up if you look at the rotor arm this should be no 1
check firing order and away you go
My 91 dodge 318 4x4 starts but falls on face when
check for rusted leaking fuel lines in back near fuel tank , clogged fuel filter, low pressure in fuel pump check with gage , check air filter , loose plug wires , bad plugs and wires , timming, cap rotor, bad gas from the botton of the tank, see if you can get a code out of it that would help alot
Disconnecting the quick fuel line connection
If your talking about the one with a plastic collar that is supposed to push in to release. They get dirt, salt, and rust in the place where the plastic ring pushes into.Use plenty of penetrating oil and compressed air to blow out debris. If all else fails you can cut the end off and use band[fuel injection] clamp to hold onto pipe .
