Hello. It seems the 'shift fork' is either 'misaligned' and/or 'bent' in 3rd, 4th, 5th and REVERSE. It's, also, possible those gears are 'stripped' on their shaft(s); making driving that truck, in those gears, impossible. Good luck and keep me informed; if you want to.
Hi Keith. I'm Tim Dorton. It's hard to tell without using a lab scope. Is it making noise?If so, then the bearing inside are about to go.If running bad, the Cam sensor is inside. In either case, check a local junk yard, there are a few distributors around. At least swapping it out can tell you if that's the problem. did you reprogram the New ECU to the Truck ? if not there is your Problem , if crank or cam sensors fail to operate ? this truck will not crank at all , the Distributor in this year Model truck is old school mechanical type and they are the safest in market . they have internal mechanical driven pulse ignition modules if these modules fail ? you will have crank but no spark without ground short . you must Have either unprogrammed ECU ? or a wiring short in main Harness ( between power and ground ) to cause ECU to fry , Ignition non these trucks are also direct power supply with 40 Amp .

These are Two separate circuits, if the tail lights work the tag light bulbs are probably burned out. and as the other info said look at the linkage that moves with the shifter and the reverse switch is part of a larger switch that also contains the neutral safety switch that allows the vehicle to start only in park or neutral. The ECU is usually the last thing a mechanic would suspect for a problem, after eliminating other possibilities. And you should concentrate on what happened to the fuel pressure-why did the fuel pump stop working. So you need to check and test the fuel pump circuit.
Look for the fuel pump fuse first of all and check that it is good. Here is how most fuel pump circuits work: The fuse supplies power to the fuel pump relay (under the hood), and when the relay is activated (or energized) by the ecu signal that goes to the coil side of the relay, then the fused power passes on through the relay and to the fuel pump in the gas tank.
If you have no fuel pressure, the pump has quit working. So to test the circuit, you would use a test light or volt meter at the fuel tank electrical connector. Find the wire for the fuel pump and see if it has power on it when the engine is cranking over. If it does have power, then you need to pull the pump off and replace it. If there is no power to the pump when cranking the engine, the pump circuit must be the problem. Better find a wiring diagram, you will need it to diagnose further.
some high pitch whistles can be from the brushes of the alternator needing attention

Your transmission is obviously failing, but it may be something as simple as low fluid level or the need for a standard transmission service. Has the fluid and filter been changed in the past 50,000 miles? If not, START THERE. It may be too late to save the transmission but there is always a chance you may.
Many parts stores have loan-a-tool programs that will include fuel pressure test kits. Many cars are equipped with a schrader valve type port on the fuel rail to make testing easy.
you do not say wether check engine lite is on or if there are codes, the fuel pump may be the culprit and be worn out and getting lazy, which can cause this problem.
Fuel pump pressure can be checked with a gauge to verify condition of pump.
