Replace the mas air flow sensor. There's a resistor it must have blown already that's why it's not calculating the right amount of air. Also check to confirm if the air cleaner housing is ohk.

It's on the throttle body where the intake manifold is this is round and has a plug facing up and 2 bolts where it attaches with a diamond shaped base

I don't see that engine listed for that year make or model. Are you in the U.S.?

Any number of thing's could cause this condition , Weak fuel pump , low fuel volume , an engine sensor , mass air flow for one , A crank sensor that is going bad , cam sensor etc... your best bet would be to have it diagnosed professionally , rather then guessing an just replacing parts

It says the Code is for a fuel additive. This does not sound like something for a normal Ford Focus. The Code could be wrong as submitted, or there are some Blu-tec style cars with diesels using a Urea additive that needs replenishing when low.

Looks like a Body Control Module communication issue. Windshield Washer When the washer switch is pulled towards the driver, the SASM sends the washer request to the BCM over the LIN. The BCM activates the windshield wash relay (internal to the BCM) and sends a message to the driver side windshield wiper motor. When the BCM activates the windshield wash relay, voltage is provided to the washer pump, directing washer fluid to the windshield. For strangeness like that you will need an auto-electrician with a pro grade scan tool computer. It is likely to be a Body Control Module issue.

It is broken and it is not a simple fix. The simpleist you can get is pulling the fuse to it for 24 hours in the hope it might reset. Unless someone has connected an FM Radio aftermarket to the Auxilliary Input lead. Or if it is Bluetooth enabled and you have a phone, is it playing your phone playlist? That happens to me.
