This unit should still be covered under warranty. I would contact Numark for an RMA.

You might have a centering control among the options of your monitor. Look up the manual, or just play with the buttons until you find it.

You may have dirty lens. You can clean it using CD lens cleaner or clean it manually.
If no joy, your optical lens may defective and need replacement.

Other possiblity cause is the mechanism is out of service.

Check your cable again. Cables for patch panels are DIFFERENT. The tip on a patch cable is smaller in diameter and has a point on it. A 1/4 inch TRS cable's tip is larger and has a rounded tip. If you're sure of your cables, then replacing the jack in the mixer isn't so bad if you're handy with a soldering iron.
