Audio does not match the video
I have some bad new if you go to google and type in "Motorola DCH3416 lagging audio" you will see this is a common issue with the lag being upward of 10 to 60 seconds. No solution at this time has been found.

TV will not display picture or sound except for 1
DONT USE the remote that came with your TV
use the buttons on the box to change the channel. did the channel change?
usually when someone only has one channel its because they are changing the tv channel not the cable box channel
think of it like this:
the TV is a monitor and should always be on the same channel (3, 4, hdmi, or whatever) its actually the box that tunes and the tv always stays on the same channel.
so if you change the tv channel you are no longer watching the output of the cable box.
USE the remote supplied by your cable provider

Dch3416 cable display zero??
the box is missing all its channel line up info
best case sanario in 1/2 hour it will be done downloading it all again. too impatient to wait?
1) turn everything on as if it was working.
2) press power to turn off the cable box only (leave the tv on) and begin pressing ok or select over and over right away. may take a couple tries
3) you see a white screen with black text? select OOB and see if it says good or at least fair
4) select inband and see if it says good or atleast fair
5) if both good or fair wait; else make certain all cables are tight then check again
6) if they are NA or poor or anything of the sort call your cable operator

Doesn't receive signal from remote control
if it works intermittenly then the remote dosent need reprogrammed. SOoooo....
line of site comes to mind. your box needs to see what your remote shoots out. sometimes something ends up in front of the reciever spot on the cable box (FYI: a flashlight at the right angle can help find the sweet spot on most devices)
maybe the dvr is going bad watch EVERYTHING you want to see one last time and have it swapped (like death, you can't take anything with you)

How to find motorola cable box manufacturer number
If it for some reason is not in the back of the cable box or underneath, just put in the model number in google search and you should find it there... hopes this answers your question, good luck.
