Open Tray Error can be caused by a couple of things.
Here are the most likely culprits:
Process 1: Clean Tray Drive Belt
Difficulty Level : EASY
Allow tray to open, then unplug with tray still open.
Flip Xbox over.
Removing front cover helps with access: If you look carefully, you can see the drive belt (small black belt). Use cotton swabs dipped in alcohol to carefully clean all dust and debris from the belt. You can (CAREFULLY) move the tray back and forth to ensure all of the belt is cleaned.
Once done, plug it back in and allow to reboot completely.
Once rebooted, open tray and insert disk to see if error is solved.
If not, then proceed to the next process:
Process 2: Adjust Laser Power Level
Difficulty Level: Expert
I have done this on my and my daughters 360's. You'll need specialized tools to open the case. If you don't have them, please don't butcher your Xbox360 using screwdrivers and such. Order what you need and do it right. The process is too detailed to describe here, so check these links instead.
I hope this helps

If you have forgotten your pass code for your Xbox 360, you can reset it by answering the pass code question you created when you set up parental controls.
If you don't remember your pass code question or don't know the pass code for your Xbox 360, you can reset the pass code online. Here's how.
Note You must meet the following criteria to be able to reset your pass code online:
Is it every single disc you have or only 1? If it is every single disc then your disc reader could be dirty or dusty. If it isn't every disc you need a new disc. Try DVDs as well. Make sure to try 5-10 other discs. If you play one disc way to much and the system does't get proper cooling you can actually burn the CD sometimes. It is very very rare but it is a possibility. If none of these things solve your problem then you need to look into a new console because it means you disc head reader is shot and needs to be replaced but that costs twice as much as a new 360 elite.
It sounds like your hard drive is toast.
More than likely you've lost everything on it. If you were sync'd with the cloud, then you should still be OK, otherwise, your data will be hard to recover.
You can open the hard drive case, and check the internal connections as it could be as simple as a loose connection. If the case has never been opened, then you'll find one security screw hidden under the Microsoft logo.
