How to create vlan
Virtual Local Area Network (VLAN) technology logically divides a physical LAN into multiple broadcast domains, each of which is called a VLAN.
Each VLAN functions as a separate broadcast domain, with devices in the same VLAN able to directly communicate with one another, while those in different VLANs cannot. As a result, broadcast packets are confined within a single VLAN.
Certificate error
A "certificate-error" occurs when you access a "secure-HTTP" site (one that uses the 'https://' prefix for the URL) and the "certificate" that the site is "secure" (namely that the site really is the site that it claims to be) is invalid. Certificates have a "start-date" and an "end-date" and certificates can be "revoked". If the certificate is for a different web-site ('web.site.com' versus 'www.site.com') or is revoked, or if the current date is not between the "start" and "end" date, you get the "certificate error" message.

Try a brand-new cable between the modem and the "uplink" port on the router.
Try a brand-new cable from one of the "LAN" ports to your computer, and test to see that you get a good connection.
How to create vlan with
Login to the built-in web-server interface, and use the interface to add specific ports to a specific VLAN.
I forget the password of
Reset the router to "factory settings", which restores the original password for the 'admin' ID.

All the lights on the
One-by-one, disconnect one Ethernet cable, and see if the pattern changes. If so, then one computer may be creating "excessive" network traffic (file-sharing? compromised-and-sending-spam-messages?). Take that "bad" computer off the network, and do some forensics & virus-scanning.

Disconnect all the Ethernet cables from all the ports.
Connect one of those eight "bad" ports to your laptop.
See if the port works when it is the ONLY active port.
If not, move the cable to the next of the eight "bad" ports, and test again. Repeat.
Move the cable to one of the 40 "good" ports, and test.
If it works, then you have a hardware problem with that "group" of 8 ports.
Your choice to use only the 40 "good" ports, or to replace the router, if you need those last 8 ports.

The point of separate VLANs is to keep traffic on one VLAN from reaching the other VLAN. So, you asked for the two VLANs to be "isolated" from each other.

Why do you want to enable MAC-address filtering?
Do you have employees connecting "personal" or "unauthorized" computers to your network? Have them sign an "Appropriate Usage" statement, defining your company's policy that no "foreign" computers are to be connected to your corporate network.
Dead ports
Excessive heat.
Dead internal fan that no longer is removing hot air from inside the router.
Past electrical "surges" on the electric company's lines that have "fried" a post.
Physical damage from repeated plugging/unplugging of the Ethernet cables.

Disconnect all the LAN cables from the switch.
Power-off the switch.
Power-on the switch.
Connect one computer.
See if that one computer works "quickly".
If so, connect one more computer, and redo your testing.
Repeat.
