Do not enter any code into your phone. The PUK code is a code that your service provider has and not one that you would know or have in your manual. If your an AT&T customer they do provide PUK on their website. If you have entered in a code and the phone is now saying PUK blocked you'll have to go to store to purchase a new SIM card.

To Unlock KG800/LG
1. Insert sim from the network that it is locked to.
2. Turn on phone
3. On home Screen, Slide up phone
4. Type 2945#*#
5. ENG. Mode menu should appear
6. Go Down to Factory Reset Select it, may take a few minutes to reset!!!!!
7. This will reset the phone back to before the Phone was locked to a network
8. The phone will automaticaly Restart(Note The Network Logo At start up will not change)
9. Now insert Any simcard(Not tested with 3 sims)and the phone should turn on normaly
10. The phone is now unlocked and you can customise the setting as you like.

I have had a similar problem with one of my phones like this a while ago. I would ring up your network provider, or which ever network your sim card is registered to and tell them the situation that your sim is blocked because the puk was entered incorrectly, they then may ask you for a few details off you just to verify that you are the owner and then (i think) should be able to unblock it or will let you reset your puk code.
