Not knowing the model # makes it difficult to give you a difinitive answer. Kingston's support docs all say that after x mny failed logins the device will have to be formatted. The default # of failed logins seem to be 10. I think you're out of luck.

I have seen this many times, most of the time it's because the index file was writing or cleaning itself when you removed it. This is what the "EJECT" is when you right click.
