Here is the recipe from the manual

3 cups of water (80 to 90 F)
3 tbsp butter or margarine, cut into pieces
1 tsp salt
3 tbsp dry milk powder
4 tbsp sugar
5 1/2 cups bread flour
1 1/4 tsp active dry or bread machine yeast

1. Measure ingredients into bread pan in the order listed
2. Insert bread pan into unit, close lid. Plug unit into wall outlet
3. Select Sandwich setting and appropriate loaf size (3 lb)
4. Press start button
5. The Complete Signal will sound when bread is done.
6. Using pot holders remove bread pan from the unit and carefully remove bread from pan and turn right side up. (Kneading blade may remain in bread. Remove blade when bread is cooled.
7. Allow bread to cool on wire rack until ready to serve at least 20 minutes.

Either the inner pan is not seated deep enough, or the bushing where the paddle comes through the pan has raised up a bit. Support - Parts for replacement parts on the Black and Decker website.

NO, it;s better to wait until kneading has started. If it has a second kneading add then. If not, add in a few minutes after ingredients have started mixing. Just lift the lid and drop them in as it mixes.

Cheaper to buy a new waffle maker

There is a sealed bearing in the bottom of the bread pan. The paddle sits on the sealed bearing shaft. Over time the bearing begins to fail and the paddle does not turn freely or it may even freeze up. If you are sure everything is installed correctly this may be the problem. See if the paddle moves freely by hand and is not really loose. Sealed bearings can not be replaced. Eventually you will need a new bread pan. Sorry Hank

I hate to say it but it is time to replace it and Cuisinart mixer a very good steamer with glass stainless steel insert and removable water tank

Recommend you go ahead and put your raisins in with all the rest of the ingredients in the beginning. Raisins are pretty tough so I don't think they will just puree. Please let me know if this solve the problem.

Shortly after closing the waffle iron it will start to steam, Watch when the steam stops and open the iron. The waffle should be golden brown. After you get the hang if it you can adjust the cooking time for lighter or darker waffles. There are videos on Youtube that may help too.
