Hi,
This is a common problem on these models.You need to contact Lexar and ask for firmware updates. Once you install them, this error would be removed.They have a firmware patch for the same.

Hi there.
Connect it to pc and find it in 'my computer', open it up. Click on
view/select all and move the songs to your pc. Right click the player
icon, select format/FAT, ignore 'Quick format' , then virus scan all
your songs, and move them back onto player.

Search for a reset hole on the device. Then reset it using an unfolded paper clip. Then try to connect and reformat it again in the computer.
Try this:
1. select RUN from start menu and type cmd
2. from there type "chkdsk F: /f"
see what happens.

In my computer I see Removable Disk (F:), when trying to open it says it is not accessible. The request could not be performed because of an I/O device error.
Can't find a reset hole. Still getting the same message from my computer. Screen on device says it is connected to the computer. I do find the device in device manager and it says the device is working properly.
It says the filename, directory name, or volume label syntax is incorrect.
I typed cmd2. hit return, then typed "chkdsk F: /f"
