The factory Admin info is Polycom for the user and 456 for the password. If the password has been changed, you can do a factory reset by holding down 4 keys until it restarts. The four keys are * 4 6 8.
To reset to factory default settings.
1. remove the power cable from the phone
2. press and hold down the * key, then put the power cable back in
3. let go off the * key
4. press # 5 * 5
5. the phone will revert to its factory default settings
The administrator password to login to the phone's web-configuration page will have reverted to one of the following default admin passwords:

note: For the [blank] password, just press the 'login' button when nothing is typed in the password box.
On firmware version 1.36, if the "iptype" setting is set to the "modem" option, the phone will crash at bootup, showing "Booting.." on the LCD display. To fix this, you will have to boot the phone in safe-mode, and change the iptype setting out of 'modem'. To do this...
1.Unplug the power and Press " * " and then plug in the power while you holding the " *" key. When you see it show " * " at the G668 LCD screen, disconnect the power.
2.Repeat Step 1.
3.Then, while the power is turned off, press and hold the "#" key and then plug the power again.
4.The G668 LCD screen should show "Password", please key in "1234", then the "speaker -free button"
5.The screen will go to "network settings", please press "speaker -free button", and then it will show "iptype 3[modem]" or whatever in your setting.
6.Press "Local IP", the "iptype 3[modem]" will become "iptype _" (the underscore is the cursor)
7.Press "1" for dhcp or "0" for static ip (recommended to press "1" for DHCP for now) and then press "Redial button" three times to the "save settings" and then press "speaker-free buttorn"
8.The screen will show "save Are you sure?", please press "speaker-free buttorn" again. G668 will reboot and work normally.
