The right earphone holds the batteries. Grab the cushion and TURN it slightly and it will release from the headset. Don't pull it off without turning first. The two AAA batteries are exposed. Just replace and replace cushion with slight turn.
You could look around in your town/city in the dollar store. they sometime sell some there OR even if you don't feel like searching to much . Buy a cheap pair of Headset at the dollar store and remove the foam piece from it and put it on you headset

I too have had the same problem. I was sent to a local Radio Shack but found this site online which seems to be a very good deal and worth trying. I googled - Ear cushions for the H12 Bluetooth Headset basedon info from Motorola and found the Cell Phone Shop. They have pkgs of 6 silicone earbud covers. On special for $3.99 and mail shipping is $4.99. Motorola will sell 3 of the foam things for $9.99 if they ever come in stock so I'm going with these and I get 6 and they fit over the earbud. They can be returned for refund if they don't work.
