Don't try to enter a PUK code without knowing the right one. You need to call your service provider to get this code. It can be as many as 8 digits, and it is random. If you enter too many wrong codes, you can permanently damage the device or the sim.
I had the same problm as you just this morning. I phoned up my network supplier and they guided me through every step and were very helpful. So you should do the same.
You need to call your network provider and they should tell you what the PUK code is or reset your phone. it is free. its happened to me before and cause im with 02 you can just go on the website and get your puk. best thing to do is call your network though.

Every PUK code is unique so nobody can tell you your PUK number, but i will tell you what you can do!
1)
If you still have your packet that your Sim Card came in, you should
find your PUK number in there, if you don't have the packet, please
read bellow.
2) You must get a phone that is on the same network
as your phone and call the networks Customer Care and ask them for your
PUK number, they will ask you for your phone number, then they will
tell you your PUK number. Once you have done this, you will be able to
reset your Pin number so you will be able to go into your phone again.
