Sounds like you could be having a reading error with the disc, I recommend you try another CD if possible, if not you could download the torrent online, and use your own serial key (that way it isnt illegal, and you are able to play it without the CD)
^ you can DL the installation disk here. They are in .ISO format (a disk image) . you can use a program called PowerISO or Daemon Tools to handle these files. Comments on the page say this is a good DL, no viruses.
I had same problem, kinda along time ago posted, but I'm hoping this helps others in the process.. You have to beat the last Quest in Act5 which is ''kill baal'' you have to do it *yourself* for it to work, meaning if you used a hack to win the quest for you, it won't work. Good luck
