The PUK code is only available directly from your Network Provider. PUK is a personal unblocking key which is actually related to the Sim card and not the actual phone itself. Your Network provider is the only way to gain access to this information so you will need to contact them.
The good news is the Network provider will give you the PUK for free.
All you need to do is ring your provider (on another phone obviously) and once they have established that you are the owner of the phone they will give you the PUK for free and directions on how to enter it into your phone.
There is no universal PUK code available which you key in and get the phone unlocked. So do not try
to key in PUK codes which you are not sure of, in an effort to unlock
the phone, because keying in the wrong PUK code, 3 or 4 times in a row will
permanently deactivate your SIM card.
So to unlock your Nokia's PUK code, you will have to call the Customer Service Helpline of your Network Service Provider, who will ask you for the Registration Number of your

I've also come across several blogs that mention placing the cell phone's battery in water for about 5 to 10 minutes and then drying it and then on placing it back in the phone, it never asks for a PUK. Do not enter any code into your phone. The PUK code is a code that your service provider has and not one that you would know or have in your manual. If your an AT&T customer they do provide PUK on their website. If you have entered in a code and the phone is now saying PUK blocked you'll have to go to store to purchase a new SIM card.

Hi PUK is when you have incorrectly entered your SIM's pin number. After 3 failed attempts it will start asking for the PUK code before the phone can start up. If you keep on entering PIN incorrectly a further 10 times it will permanently block the SIM, which is irreversible! If you have on screen PUK BLOCKED, I expect you have gone too far. You will need to speak to your network operator and see if they can replace your SIM for you, but I am afraid you would have lost any numbers in your address book that was stored on the sim!
