I guess it cant able to read any of your disk and this will occur only when u hav som problem with settings or the disk reader problems..u can try this site to solve your problem..

Try going to download it again. XboxLive/ PSN can tell you already bought it so it wont charge you again, but it WILL download everything. Problem solved
My guess is you have a "crappy video card" that's not capable of playing CALL OF DUTY BLACK OPS. Check the system requirements @ http://www.systemrequirementslab.com/cyri/
The XBox 360 is a wonderful video
game console, however, with heavy usage, the laser (and/or other components in
the drive) will eventually burn out.
The first signs of the laser starting to burn out is that your game will start
freezing during game play. Eventually the games will stop reading altogether
and you will get an "OPEN TRAY" message when you put a disc in and close the
tray. What this means is that the XBox 360 thinks that there is no disc in
the system.

Sign into a PSN account and launch Call of Duty: Black Ops III. After reaching the Main Menu, connect a second controller. Press X on the second controller to connect the other player online. Once connected, press X again to attach the split screen player.
Unfortunately that particular controller has been noted for it's somewhat spotty performance. You may just have one of the many lemons for that product, so I suggest first taking the product, along with your receipt, back to where you bought it and asking for an exchange. (You did save the package, right? Maybe you don't need it if your just going to exchange it for another, but better safe then sorry.)
As for a long term solution, check online before you buy an aftermarket product like that. Reading the reviews on a place like Amazon will give you an idea of the product, and a list of things that are good and bad about it before you go out and buy one, either online or in a store. There are plenty of good items for the PS3, but there are some that haven't had enough work put into them yet.

open the dvd drive up and grease the positioning rod, then push the laser all the way back until it clicks. then push it all the way to the spining mech. after this is done reassemble the xbox and play =-)
