I've been trying to find someone at DISH to tell that I discovered the problem and solution after over a year of nobody at DISH helping whatsoever (three receiver changes, four live techs and several on the phone):
It was what I suspected and thought I'd actually tried very early on. I had to bypass the amp for video purposes in order for the DISH receiver to detect a signal. It would go to sleep and never wake up (in time for a recording for example0 if it was connected thru the receiver. This particular Onkyo AMP has what they call a passive pass-thru HDMI and that may make the difference, but surely SOMEONE at DISH should understand this **** better than a layperson like me! Wouldn't you agree?
