Your phone will ask for PUK code when you entered the PIN code wrongly several times depending upon the mobile make. The mobile asks for PUK code so that unauthorized persons do not use your mobile for wrong cause.
To get PUK code please contact your service provider through phone or go directly to their office. They will provide you the PUK code for your mobile. If you attempt to enter wrong PIN code more than 3 times, then your pin code will be blocked and you need to enter PUK code to unlock it.

You can get your PUK code by calling to your mobile operator's customer care.

In you attempt to enter wrong PUK code for many times, then your whole SIM card will be blocked and you cannot unlock it. Getting a duplicate SIM card is the only solution if your SIM card is blocked.
