This is usually caused by a bad bearing on the shaft of the Gear and Sprocket Assembly. When it wears enough it will also let the chain come off the sprocket. If you look at the sprocket closely you can usually see that the shaft is leaning toward the door.

Sounds like your drive gear has stripped. Remove the cover from the motor unit and check the gear. It is white nylon and it will be obvious if it is bad. Sounds like a good time to take a good look at your door springs. That loud bang may have been one of the breaking.

Sounds like the gears might be out of the operator. If you can remove the cover see if there are white shavings in the cover. A Craftsman operator will take gear kit # 41C4220A
