Its not the phone, but the sim that requires a PUK code. If you check your original sim starter pack, you would see your PUK code printed on it. I want you to know that its quite different fron the four digit pin code. Enter this puk code to unlock your sim. But if you cant find your starter pack, then I want to believe you know the number of you customer care, you could ask one or two people if you dont. Then, when you know the number, use some one else's line, but of your same network, perhaps a friend's, and call the customer care agents. If you don't know the number of your cellphone you can tell the number present in the SIM card to get the PUK Code. for the puk code you need to phone your service provider and ask for it , they will ask you some questions and they will give you the code. It seems you entered the wrong pin number more than 3 times that is the reason you need to answer those security questions !a nice tip is to call them from a phone box and tell them you don`t have much money and they will phone you back most of times if you cant do it another way !
PUK code can be given only by your service provider which is specific for each and every mobile number. Contact your sim customer care department over phone. Its free of cost to get the pin or puk no. If you cant contact through your phone get some other phone and contact them. 1) If you still have your packet that your Sim Card came in, you should find your PUK number in there, if you don't have the packet, please read bellow.
2) You must get a phone that is on the same network as your phone and call the networks Customer Care and ask them for your PUK number, they will ask you for your phone number, then they will tell you your PUK number. Once you have done this, you will be able to reset your Pin number so you will be able to go into your phone again.

So, when it asks for the PUK code, don’t do anything, get the PUK code from your service provider. Call to customer care using any other mobile phone which uses the same service provider. They will ask some details about you to make sure you are the owner of the SIM card. Questions might include your billing address, email ID, numbers you recently called or messaged etc. They might even ask you number printed on the SIM card, so be prepared with all details before calling them.
