Well your disc is officially garbage because it's for sure scratched. Don't bother trying to fix your game disc. Do what I did. Go to sellitbroken.com and sell your game to them. They will give you good money for it.

firstly its refurbished so you have to be careful because it may have had problems in the past, it referring to your hard drive by the way. ad secondly maybe its just your black ops disc? try playing black ops on another xbox console and if it freezes than its not your console its just the game =) then get a replacement and dont worry.
it sounds like an error with you gpu chip you need to google x clamp repairs and buy a x clamp repair kit from ebay this should solve the graphics error
YOUR DISC COULD BE DIRTY. THAT WILL ALWAYS CAUSE GAME FREEZE. CORUPT FILES WILL NEVER ALOW SMOOTH PLAY. TRY EREASING GAMESAVES AND EREASE ALL UPDATES, INSTRUCTIONS AT YOUTUBE.COM REINSTALL UPDATES. START A NEW SAVE. PLAY ON. HOPFULLY THESE WILL WORK. DO ALL OF THIS BEFORE GOING TO XBOX.COM IN SUPPORT USE THE DISC REPLACEMENT PROGRAM. ITS FREE.
