Sounds like you've used this thing quite a bit, eh? Rechargeable batteries only last for so many charges. If a charge doesn't last very long anymore, you probably need to change the battery. This is not for the technically challenged. The battery may just pop out of a slide cover, or may require practically breaking the device in half to get it open, then unsoldering the old battery and resoldering a new battery in place.
But, it definitely sounds like you need to replace the battery. You may have to consider a new one - sorry.
Press and hold the button on the Jabra for 10 seconds until the LED starts blinking. This puts the headset in "discoverable" mode so the new cell phone will be able to see it for pairing.
If your phone prompts you for a pairing code, try either 0000 (four zeros) or 1234
Try this.. depending on the Jabra hold the button for a few seconds until the light goes blue or if there is a pin hole on the jabra
Using a paperclip, lightly press into the hole till you feel a click. The display will change to say pairing mode. Go to your phone and you will see a display asking permission to pair. The password is 0000. Once it is in, go to the selection of Jabra XXX and press on the trackball. Go to DEVICE PROPERTIES and press trackball. Go down to TRUSTED and press trackball. Roll trackball to YES.

This is one of the mistake commonly done by many people. First you need to pair. Let me walk you through. First, the headset has to be switched to pairing mode which is done by holding the on button for a while even after the headset is switched on. Then the bluetooth headset light will remain on. You switch on the phone bluetooth, search new device. Then select it and press the option for pairing. Read the instructions which come on the phone display and select accordingly. It will ask for passcode which is usually four zeros. Then it will get paired ask you whether to connect. You select connect and you are done.
