The solution posted by dick_shivers did not work for me. Here's what did. Do the Mac Address reset like suggested, however, do not let the phone fully boot. Once you are able to press the setup button, key in the 456 password (previously didn't work, but now should). This will not work if you let the phone boot all the way. Now that you have access to the Admin menu you can disable the DHCP server option and assign an IP phones address, Subnet Mask, and IP Gateway. Then under server menu, enter FTP server (FileZilla) address, user, and password to upload the sip and bootrom files for the phone. I know the FTP server menu stuff is vague, but it's too much to include in this post.
Plug the phone in and allow it to boot up. Once the home screen is displayed, press and hold the 4, 6, 8, and * keys for about three seconds, or until you hear an audio alert and the phone asks you for a password. Enter 456 (default Admin password) and the phone will default itself. If the password has been changed, and 456 does not work, enter the MAC address of the phone as the password. Entering the MAC address of the phone will only reset the Admin password to the default, so it will probably be necessary to repeat this procedure, entering 456 the second time, to factory default the phone.
