I logged on to the Actiontec modem/router with 192.168.1.1 so I am assuming the Gateway is the same. My DVR has a static IP of 192.168.1.200 and a streaming port of 8000. When I connect my laptop to the LAN, the Actiontec modem sees it. However, when I connect my DVR, it does not see it.
I created a port forwarding rule and now I want to assign that rule to the DVR. However, Actiontec (apparently) only allows me to assign the rule to the recognized devices shown in a pull down. Am I able to create a device with a static IP Address or how do I get Actiontec to recognize my DVR?
The DHCP range used to be from 192.168.1.64 thru 192.168.1.254, so I changed the end range to .100 but the DVR still didn't show up.
I have another Actiontec modem that allows me to create my own devices with an IP Address.
Since my laptop appears and disappears (Actiontec recognized devices) when I connect it to the LAN, I did not reboot the DVR or modem.
Please advise.

I think the screen you may be looking at is the Application layer forwarding screen, the drop down box being populated by DHCP clients, that's why it isn't seeing the DVR. The DMZ Hosting screen doesn't have a drop down box, it only requires an IP address to be entered.

Sad to say it, but Actiontec Modems seem to have a huge lacking when it comes to configureability of the firewall rules and multiple port forwardings. All my dealings with Actiontec have left me to believe that the firewall portion shuts down communications when a single incoming IP address attempts access to several ports. Actiontec even has a system for VPN Pass-thru, but even it got clamped. My only real suggestion on this matter would be to purchase a standalone firewall "brick", shutdown the firewall on the modem, and DMZ host the "brick" and let it manage all the traffic.

You can use USB instead of Ethernet port but its not a common practise. Leave it empty then.

You'll probably need to turn on the wireless inside the configuration. go to 192.168.0.1 in your browser and head to advanced. From there you'll see it :)

Yes, if the other isp is also providing dsl services, but you will have to configure the device the other isp will not help you with that.

Thank-you for your willingness to help. The modem is actually a GT704-WG. My laptop is set DHCP; the DVR has a static IP. The PC and DVR are plugged directly into ports on the modem. There are also wireless PCs and they show up on the Network as seen by the modem. My goal, of course, is to be able to see (Telnet) the DVR from the Internet.

The DMZ host screen on my GT704-WG has the same pull down selection menu. I rebooted the DVR and the modem. Perhaps, I need to update the firmware? Maybe I just need to get a new modem.
The firmware rev is 3.20.3.3.5.0.9.15

I want to thank-you for patience and kind support. I figured out how to port forward to my DVR using the GT704 Actiontec modem. GT704 will show all the DHCP devices but not static, even though the IP Address is of the same subnet. I configured the DVR to DHCP and it showed up as part of the Actiontec network, listed as "unknown".
"unknown" (my DVR) showed up in the pull down menu along with the other DHCP devices.
Then under Security/Applications/User I created a new rule for the port set in my DVR. (Port Forwarding is called a rule). Using the pull down menu of DHCP devices, I was then able to assign the rule to my DVR. Then everything worked.
Since Actiontec assigns rules to a Host name and not to a specific IP Address, it doesn't matter if the device IP Addresses are reassigned during a reboot of the modem.

I seem to have the same problem.
I have an Actiontec GT704-WG and I am trying to make my Tivo HD work with the unit. I have tried to create a port forwarding rule, but I DO NOT know the syntax.
EXACTLY how does one go about setting up the rule? The ports that I need to open are: TCP --> 37,443,2190,4430,7287,7288,8000,8080-8089,8101,8102 & 8200. UDP --> 123 & 2190.
All computers & Tivo's use DHCP to connect to the GT704-WG router. The Tivo DOES show up in the "Security --> Applications --> (pull down).
For example, to open TCP port 37 I entered a user defined application rule with a rule name of "TivoTCP" and Port Start = 37, Port End = 37, and Port Map Start = 0 (which seems to change it to 37).
Yet after adding all of the ports (TCP & UDP) and applying the rule to the Tivo connection, it will still not communicate with the Tivo servers. I must manually turn off the firewall in order for it to communicate.
Any help would be great.
2 items that I need clarified,
1) Is your Laptop using DHCP?
2) How is your network wired together?
Clairification on whether you have a GT701-WG (as you have posted) or GT704-WG (forum it's posted in) would be nice, but the 2 devices seem to have the same screens as my own DSL modem.
