If by the "ball" you're referring to the type of flow gauge that works by a stream of gas pushing up a small plastic ball in a clear tube or drilled piece of clear plastic, then the ball is either stuck somehow (moisture in its tube might do that by causing some friction), or it's showing there's no flow of oxygen enriched air, meaning the concentrator itself is not working. Can you feel some air flow if you hold the outlet towards the skin on your cheek?

This definitely should not be used until it is serviced. There are (or should be) two black foam filters on either side. Simply remove any attachments and the screws along the bottom of the cover. The cover will lift off. Vacuum out the unit, get rid of all dust and particles. You will see an upright somewhat cylindrical filter. It will unscrew from the bottom. Remove and replace. The new filter will have the holes in it covered with stickers. Different stickers are removed and uncovering holes based on the model you are using. The filter will have the directions on the stickers. There is also a smaller round diaphragm-like filter beside the previous filter and behind the control panel. Remove holding clamps, pull it free of the tubing and replace.
Replacement filter kits are available online. I usually get mine from HD Supply. Individual filters are available but I usually get the kit, the exception being the outside filters will need to be cleaned or replaced more often. I check the inside filters every 6 months or less.

the plug in unit is probably drawing more amps than the lighter socket will cater for. Check that the fault is not in the unit. Check for total current draw of the machine. As a last resort use the machine directly coupled to the battery
