PUK code is a security code unique to your sim card. It is used for unblocking the SIM card if its being blocked by some reason. Your sim card seems to be blocked and you need to provide the PUK code to use it again.
Usually the PUK code is written on the papers you got from your provider when buying the SIM card. If you can't find it, just call your SIM card provider or Service provider helpline to get it. There is no charge for retrieving PUK code.

Hi the PUK code is the security code for the sim card its on their to stop people stealing your numbers, the only place you can get the PUK code from is your service provider.
All you need to do is give them a call and give them the phone number of the sim card thats asking for the PUK code or give them the number written on the sim card, they will then give you the 10digit PUK code.
your service provider dont charge for this service and will give you the code over the phone for free.

now the other option you have is if you dont need to use that sim card then you could go and get a new sim card and put this in your phone, also when you buy a new or get a new sim card you will be given the PUK code it will be written on the card the sim card is stuck to.

their is no other way of getting the PUK code as its only help by the person who owns the sim card and the service provider so you will need to give them a call to get the code but it is free and they will give it you over the phone.

some service providers will give you the PUK code via their web site but not all service providers offer this service so you will need to check this.

PUK or pin unblocking key, usually comes up together with your sim card, be it on a paper, receipt or at the back of your sim card card. it is usually the unblocking key for your pin. normally, puk is for your sim card not to your phone, but, if the phone depends more on the sim, either if you remove the sim and open your phone, it will not open. usually, this is possible for phone who don't have an offline mode. if you lost your PUK, then, the possible way to acquire this, is to contact your network provider and ask for your puk code. if your phone has an offline mode, try inserting a sim that the pin is not activated and check.
