Hi your only option is to call your phone service provider. No it doesn't come with the phone and the phone never has its own puk code because the puk code is about the sim card.
if you put in another sim card the phone will work just fine, its the sim card 3 times pin code wrong puk code enabled pin blocked, call service provider to get it and entry instructions.

PUK (Pin Unlock Key) is required when a customer enters an incorrect PIN 3 times. After entering the wrong PIN 3 times, the card will be locked and the customer will need to get a PUK code from operator to unlock it. Once a PUK has been received it will need to be entered to set a new PIN code.

PUK code can be given only by your service provider which is specific for each and every mobile number.
Contact your sim customer care department over phone. Its free of cost to get the pin or puk no. If you cant contact through your phone get some other phone and contact them.
Go to the nearest service outlet of your service provider they can also help you.
There is no other go than the above procedure only your service provider can give it nobody else in this world nor any software.

Hi,
If you need the PUK ( Personal Unblocking Key ) for your phone, you will have to contact your mobile phone service provider.
PUK code is related to the SIM card you are using. So, kindly call the Customer support of your mobile service provider and they will provide you with the PUK code.
Also, entering a wrong PUK code ten times in a row disables the SIM card permanently. So, please be careful while entering the PUK code.

If you enter PIN or PIN2 code incorrectly three times in succession, you are asked for the PUK or PUK2 code. If you do not have them, contact your service provider. The module PIN is required to access the information in the security module of your SIM card. The signing PIN may be required for the digital signature. The barring password is required when using the call barring service. To set= how your phone uses the access codes and security settings, select Menu> Settings> Security.
