In America, you make application through the State where you were born, usually through Vital Records. They will ask for pertinent information and there will be a fee. Always remember, state websites use “.gov”. Don’t get caught up using Ad sites, they can be deceiving.
If in UK. You need to apply to the council registered office

If you were in Australia, you would have to request such from a department within the state where you were born.
