This occurs mostly on the bottom side, which usually means tension or burs on needle, or something else from the upper side, but cannot seem to find the reason. Not only did I do a one over on top and bottom, replaced the rotary hook, tension block, needle bar, is STILL doing it...... I've sewn and worked on my own machines for 15 plus years and always worked it out until now. Help?
Set the top tension to four. Now, if the bottom seems too loose go ahead and loosen the bobbin tension a little at a time. If your upper tension isn't real true at four you may have to slightly raise the tension control to offset this.
Testimonial: "Ugh, I def did every tension setting along with even replacing the tension casing, but after pulling all my hair out, I thought, hmmmmmm, whats different? Aaaah, the presser foot! I have no idea why, but when I changed it back to the original, BOYOYOING!The only assumption I have for the issue is that the new foot was about 2 mm thicker and messed with the looper somehow. Thank you so very much for the help!"
SOURCE: Loose stitching underneath. HOW CAN I FIX!!!!
Sounds as though you may have a burr or rough spot on the hook where the bobbin inserts. Run your finger along the edge of the hook to see if there are any rough spots.I'm sure you're going to find at least one from the time you broke the needle.Once you locate it, remove it with an emery stick or small sharpening stone. Polish it so that it is smooth. Should work okay.

There is a free download threading diagram for the DDL-227 here:

SOURCE: Juki HZL-E61: cannot get tension right on straight stitch
Loops on the bottom of the fabric direct the problem to the top thread. Rethread again and make sure the thread goes through the tension discs. Lift the foot, because this is what opens the discs. When sewing and the foot is down, the discs should be closed and thus giving the upper thread the tension it needs.

Don't forget there is a "hidden" threading phole all the to left side of lower looper before it gets threaded front to back in the threading hole on horizontal lower looper that is visible looking at machine head on.
Hope this helps.

SOURCE: I would like instructions on how to set different stitch lengths on my Juki 1541-S.
Puckering is typically NOT a tension problem. But rather a bad needle or not the right needle for the project and thread. Sometimes it's uneven feed but since your 1541 is a walking foot you might have to get the feed timed. I know little about that.
I would first look at the recommended needle thread combo for the fabric. Then the presser foot tension.
Good luck.
