We (family of 4) have been using our Rinnai RUR98 (natural gas) for 2 years now and never had any issues. The most recent rescaling was done about 2 months ago. Today while I was taking a shower, the water stopped after several minutes. I went to check the water heater and it was beeping and flashing code 12. I turned off the power and turned it back on, the error went away. I asked my kid to turn on the hot water in the shower again while I stayed by the water heater to observe. After a 10 min or so, the water heater started vibrating and making a train-like shushing noise, followed by code 12. Any idea what's going on? Thanks!
Code 12 is a flame failure.
First step in diagnosing the root of the issue is to check for any separation, obstruction, or failure in the vent system.
Second step is to use the isolator kit under the heater to flush the heat exchanger with vinegar or rectorseal calci-free.
These two things are a great starting point to working on any tankless water heater. Flame failure can be as simple as the heat exchanger being clogged not allowing the sensors to work properly or the exhaust being mixed with the air intake. If the problem persists I would suggest having a Rinnai Certified Tech look at the heater.

if you have build up in the heat exchanger it will cause the unit to use more gas to heat your water, if the gas line isn't sized properly then your going to kick the error codes off. it sounds like you have two problems, first you have build up in your heat exchanger and it needs to be flushed and second you need to get a bigger gas line to it

Error 12 is failed ignition - this can be caused by many things, but normally a low gas pressure, or no gas pressure upon ignition.
Try turning the hot tap on very slowly till till the control shows burner is lit. Then turn it on faster. If it stays going with no error, you might have a faulty regulator, not opening fast enough to meet demand. Especially common with lpg models,a lot rarer on natural gas.

Try turning the thermostat uo a small amount and see if this makes a difference. I suspect the control board is bad but I would try adjusting t-stat before I spent money on board or service call. Thank you
