belts that fly off during use are most allways caused by one or two things, (1) belt has stretched requires replacement with proper length and material belt
(2) The belt pullys have worn out , this is evedenced by belts that get frayed edges or are cut to shreds
1) Check both the top and bottom tensions for loose fine threads. Thread build up will cause the thread to break. 2) Check the faceplate for sharp edges. Sand with fine paper to smooth out the edges.
mine some times does it try not to overload the drum with washing as it doesent have enough room to move so the washing rubs against itself causing holes rips and fraying

Is this the accessory drive belt that is noisy? If so check the belt edge for frayed or exposed cords like a tire cord, if that is found you have a defective pulley Or a crank pulley that is moving outboard due to the harmonic damper coming apart, this causes the belt to not run true across the pulleys and that causes noise..
Hopefully, you did not install the pulley on the pump upside-down. (I have seen this happen before) Otherwise, the end of the pump shaft should be flush with the flange on the pulley. If it is off just a little one way or the other, it can cause fraying and severe belt noise.
