Offset errors are also caused by faulty/shorting speaker leads, and faulty speakers. A shorted tweeter or midrange speaker within a 3way speaker connected onto any of the speaker wires can also cause this error to arise. Playing the unit alot at excessive volume levels cause distorted sounds with in turn bring on the errors. Connect the unit with all speaker wires disconnected, if the error then comes up again, you have an internal circuit problem or an issue with the software. Cheers.
That error is normally a shorted speaker wire but starting the car won't cause that. Possible problem could be the dimmer wire was used as a ground by mistake. Lights come on when the car starts. Another is too high of voltage. Car rests at 12 volts but is around 14 when running. Was there a plastic piece on the red or yellow wire that got cut off during the install? They have a resistor inside that keeps the voltage down.

that d.c. output will burn the coils in your speakers with out making a sound!! you have a biasing problem
or like you said a d.c. offset problem it is best to work on this problem with out a load on the amp, this will allow you to trace back where the trouble is starting at. caps will cause this sort of problem but leaky transistors can also do this, even diodes.
this will require an experienced trouble-shooter.
