Obviously, the motor shaft is either broken or loose. Sounds like poor quality to me. Complain to the manufacturer- this item should last for years, literally. Here's their web site:

You might take it apart to see if you can tighten, glue, or fix it.

Insert the OFF peg, to the time you want the light to turn off. Set the clock to around 1 hour before the actual time of day and put the ON peg in at around 1:15, but NOT all the way in, just far enough in so you can see it will engage the post pin under the "T". Now plug it in, and adjust you time to 2:00 or whatever time it is. Just make sure you preset the 24 hour clock to around 1 hour before the actual time of day and put the ON post in near the "T", so when you do adjust the clock it will engage and turn the light off. Then depress the middle button, pull the ON post out and place it at the time of day you want the light or appliance to turn on. Push the ON post all the way in.
This was the easiest way I found to do it, on the NOMA mechanical timer.

Firstly, the extended warranty is a very cheap insurance, because the labor could be as high as $168/hr. Instead of putting $200+ in this unit, I would purchase another with 3-5 yr warranty.

Try the default password first which is eight zeroes (00000000).
If that didn't work you can try hard resetting it by pressing *2767*3855# but everything will be erased!
