Usually you have the initialization codes on the large card enclosure for the SIM.
Calling the mobile network for PUK unlock assistance will also work, but first take a note on a piece of paper of all the numbers written on the SIM Card.

When My Phone Did That, I Took It Into Metro And They Sent It In A Replaced It With A New One, The Guy At Th e counter Said That This Is A Common Thing And That Sometimes If You Download Anything From Your Phone Like From Funformobile.com Or Something, Or If Someone Sends Them To You From Websites Like That It Voids Your Waranty And It Causes That. I Hoped This Helped You, So Just Take It Into Metro And They Should Send You A New Phone Within 4-5 Buisness Days.

It is usually the last 4 digits of your phone number. If you have
forgotten your phone number or the digits are not working, take the
phone to your phone subscriber
u dial *22803 then it resets the whole phone and the ur passcode will
reset to the last 4 digit of ur phone number but u have to listen to the
voice prompts press 1 for english and it will ask u to prees 1 to
program ur handset
