Most likely cause is a faulty speaker . Very common. See other questions on Fixya where I ahve posted the solution.

When the earpiece is in your ear the outside (blue part) is actually the multifuction key. Just press the button and say the command to make a call, and push the button again to hang up a call, it took me a day or two to figure that out too.

try pushing the wire into the bottom of the display, this comes loose easily and causes these problems.
